Neapolitan vent detail

Could someone please show me or send me a detail on how the vent on the Neapolitan style oven is formed. Is it cantilevered of the front of the oven? If so how is it attached to the oven?

The BEST I've seen is a custom-cast refractory vent on a dome-style oven here:

This one is a great example.  I'm going to try to emulate it.

Well done to everybody on their vent but let's make it clear that are not Neapolitan example (the actual Neapolitan vent is very, very different).
