Tips for a stovetop pizza?

Should the flame be medium, low or high?

Does cooking this way change what the dough formulation should be?

What type of pan is ideal?  Should I use a pan cover?

Is this way of cooking pizza better suited for thinner crusts or thicker crusts?

Any good stovetop pan pizza recipes?
